A different kind of help

01 / 04

Nurse Digital Health Navigators.

Not doctors. Not basic tech support. We bridge the gap between people and an overwhelming health portal—with clinical experience, clear communication, and family education.

Your care team makes clinical decisions. We help the information, questions, and follow-through stay organized around you.

Practical, nurse-led support

Make room for the care—not the portal confusion.

From the first login to the next referral, we help put the details in order and keep questions moving to the right person.

01

Inbox triage

Sort MyChart messages, draft non-clinical replies, and flag the questions or concerns to raise with a provider.

02

Care coordination

Keep referrals, appointments, and the care calendar in view, with follow-up steps organized in one place.

03

Visit and results education

Learn how to find information in an After Visit Summary and follow lab trends without clinical interpretation.

04

Portal setup and family access

Get help with MyChart access, notifications, authorized proxy access, and practical portal navigation.

05

Ongoing chronic care support

Build a steady rhythm for check-ins, medication-list reconciliation support, care-plan follow-through, and transitions.

Our role in your care

A chaotic portal can become a clear, organized action plan.

We make the next step easier to see, while your provider remains the source for clinical guidance.

A thoughtful start

First, we listen. Then we organize.

Support begins with a conversation about the kind of help you’re looking for.

  1. 01

    Talk through the need

Tell us what feels hard to keep track of. Please keep your first email general—no health details or records.

  2. 02

Agree on a safe way to work

Before portal information is reviewed, we agree on an appropriate secure channel, consent, and authorized access.

  3. 03

    Keep next steps in view

We organize the agreed tasks, prepare provider questions, and revisit follow-through in your chosen support rhythm.

A few clear answers

Good to know before we begin.

Warm, practical support—with clear boundaries and your care team at the center.

Do Guide Care Nurses provide medical advice or interpret results?

No. We do not diagnose, recommend treatment, or clinically interpret lab results. We help you find and understand the layout of visit information, notice what needs a provider’s attention, and prepare questions for your care team.

Do you connect directly to MyChart or Epic?

We do not offer a direct MyChart or Epic integration. Portal support is arranged with you through an appropriate secure channel, with consent and authorized access agreed before any account information is reviewed. We never ask you to email a password.

Can family members be involved?

Yes. Family education and coordination can be part of the service when the client gives permission and any needed proxy access is in place.

Where do you work?

Virtual support is available nationwide. In-person support is available for select clients in greater Seattle, Snohomish County, and Marysville, Washington.

Are services covered by Health Home or insurance?

Private-pay monthly packages and hourly support are available. Guide Care Nurses is designing its coordination workflows to support potential Health Home pathways, but program participation depends on eligibility and applicable requirements. We do not claim payer approval or Health Home enrollment.
